Health declaration

I am concerned about filling this in as I read somewhere that your cruise will be cancelled if you have recently visited certain countries prior to sailing.

Could someone please post a full copy of the declaration? Thank you.

Maybe it was in the earlier days of Covid when some countries were put on a 'red list'?

Could have a visa thing? Some countries won't issue a visa if you had visited certain other countries. And, if you don't have all necessary docs for the places the cruise is visiting you will be denied boarding.

Before covid there used to be a question about visiting West Africa, Ebola.

 

IIRC there has in the past been questions about visiting countries due to Yellow Fever.
