Consent form for Minors

Can anybody tell me where I can get a In case of Emergency minor consent form and a leaving the country consent form? My Family will be taking a cruise on Easter Sunday and will be taking along my sister kids who are minors 17 and 16.

I'm not positive, but I don't know of any special "forms" for this. All we did in the past was type out a "To Whom It May Concern" letter stating that XXX has permission to make any medical decisions for XXX during the period of XXX to XXX. Then we signed and dated it, and had it notarized. I would assume that simply adding in something along the lines of "and travel with said person aboard the XXX" would suffice.

My mom is going to be watching our son for us when we cruise later this month. Is it a requirement that the letter be notarized? My pediatrician told me no, but I am seeing that a lot of you are getting your letters notarized.

My ex took my dd out of country and she had me notorize all of the consent paperwork.

get it notorized--jsut in case. More then likly you will not be asked for the letter.

Also -make sure those kids have state issued IDs for boarding

Is 18yr old considered minor on cruise ship? I know you have to be 21 to drink on board. I'm taking my daughter and her friend.

As a former notary, I'd get the letter notarized if I were you. Just go to your bank with your ID and someone there will notarize it for you. (Be sure not to sign it before you go.)
