I have one completed for each and every cruise, I take my 16 yr old GD along on, and only once was I ever ask to present it. It is required, so better get it, in case you are asked to present it. AND don't forget to bring his health insurance card. (Unfortunatly we needed one for our children on a cruise) Here is the consent letter, get it notarized. megr1125 Posted October 26, 2007 #6 Share Posted October 26, 2007 While the cruise line may or may not ask for it, Immigration may very well want to see it....and i have seen several people having 'conversations' with immigration officers because they did not have the info..... and make sure you have authorization to seek medical treatment as well..better safe than sorry.. happy cruising. Make sure that you have contacted the cruise line, airline etc and make sure that you have the proper documentation. The cruiseline "suggests" them. And you could call 10 times and ask RCCL and they will give you 10 different answers. :rolleyes: It's not the cruiseline, it's Homeland Security/customs that *may* ask for the forms.
