Assistance in finding form for Guardianship on cruise of granddaughter

Can anyone out there help me. We are traveling on the Liberty of the Seas in April with our granddaughter. I was told I needed a notorized letter giving us guardianship while on the cruise. About a year ago I remember seeing on this board a sample letter that someone had posted. I have no clue where I was on the board when I saw it. Can anyout out there assist me as to where I can find a sample one to do? You help would be greatly appreciated.

You can try the website legaldocs dot com. You fill out the form online, print it, then have it notorized. It is free. At least it was when we used it.
