Transportation in Costa Rica

I need to go from San Jose, Costa Rica to Puerto Caldera to meet a cruise. Has anyone had experience with arranging private transportation (private driver, shuttle, etc) and the cost and reliability ofthe company? If so, please post the name and email or web site.
