Here is the Link for a good shuttle service company; SAS transporation that you might want to consider.

I would not book a flight until around 12:00 or 1:00 PM.

The drive between the Fort Lauderdale Airport to the Miami Cruise Ship Terminal will range from around 30 to 45 minutes depending on traffic.

30-45 minutes is a good time estimate. I booked SAS based on a number of good reviews on this board and did NOT have a good experience. The wait at the airport was VERY LONG and the van was a stinky/smokey mess. The driver was pleasant but after the long wait and the stench the trip could not be salvaged. I would NOT recomend them.
