If you're talking about the San Gervasio ruins on the island itself, just take a taxi from the port. Really not much to see or need for separate guide service. less than 45 minutes from Puerta Maya.

 

If you are thinking about Tulum or Coba on the mainland, these are fairly long excursions and it's best to book via the ship's excursion as you must catch a ferry both ways and if you are even 1 minute late to the ferry dock on return and the ferry is still at the dock, the ticket booth may not sell you a ticket. It's an hour + wait for the next one.
