It's up to the operator, but usually 12 and under is considered a child.

The documentation we got from Celebrity for future excursions calls out age limits for certain ones. There are also height and weight restrictions so just check the details on the one you are considering. We are sailing Millennium in August 2017 and have a printout of all excursions. Was reviewing today and saw specific limits for things like zip lining, kayaking, etc.

Regarding cost as opposed to age limits on certain excursions (as others have posted above), we were told that our children would be charged the adult price when they were classified as adults from the cruise booking paperwork i.e. pretty sure it was age 12 and upwards, as per the actual reservation for the cruise. So if you prebook excursions, it will automatically show if they will be charged adult or child rate; this is the same process if you book onboard - the Excursions Desk will automatically inform you when reserving tours what the charges will be, irrespective if you ask for a child rate or not.
