I always email them at guestAdmin@carnival.com and provide them with a copy of my latest statement from the brokerage company. I just did this for my August cruise and had the OBC in 3 business days. Make sure to privide your booking number so they can figure out where you want it applied. I thought you could only snail mail or fax the information to Carnival. Also, make sure you block out your customer number for your broker And no one else has mentioned that this applies to all cruise lines owned by the Carnival corporation, not just the Carnival line.
