I was just able to fill out the Vaccine Attestation online. Please try it again.

Anyone know what to check for someone under 12, obviously not vaccinated yet. I can’t see the full answer for the last 2 selections. Can’t get through to customer service

We're waiting for 48 hours to pass.

Carnival, Please cancel our cruise so we don't to pay cancelation fees.

Will rebook later when all these issues goes away...

Is the Vaccination Attestation the same as the Vaccination Survey that I filled out when I booked a month ago for a December cruise?

I thought I read that you have to fill out another form a few days before cruise, so is that a completely different form?

The survey only asked if we were vaccinated or not. Nothing else.

The Health Assessment will be filled out a few days before your cruise date.

Combining the correct answers given by multiple into one:

The vaccine attestation (or survey, as it's labeled online) is a one word question that comes out far in advance of your cruise. The question boils down to "will you be fully vaccinated on cruise day"

 

Then, later, about 3 days prior to your particular sailing, there will be another online form, this one called the Health Assessment.  This one will be questions substantially similar to "have you in the past 14 days had a positive Covid test" and "have you in the past 14 days had close contact with a person who has tested positive"

 

Then (at least for me when I boarded the Breeze in late July), at the port, during my doc check, they asked me the same Health Assessment questions again.
