Special Needs Form

For anyone who has used this form, did you fax or email it to the information at the bottom of the form? It says to use that contact information for questions but doesn’t list any other information regarding where to send the completed form. Thank you!!!

I send it to the fax number.  Yes I am a dinosaur and still have a fax machine in the house.

I just emailed it. Simple and got a response very quickly (just a note telling me that they received the request and will respond a little later).

This was our experience as well to our request on November 5, 2021.

We have received no further response so far, but will be calling them soon.

I received a final response/answer in a couple days. Your request may have got lost
