Incorrect Data Entered on Health Control Form

A member of one of the groups I'm familiar with accidently entered their DOB into the Spain Health Control Form (FCS) incorrectly.  How do they get it corrected?  The form allows some changes but not DOB. They said they tried just starting a new one but that didn't work.
