Passenger locator form for Antwerp

On Discovery 15 April. My partner is trying to set up the form, but having difficulty understanding some of it, e.g. address etc. Seems to assume we will have a normal address, which of course we haven’t on a cruise.

I m not sure if this is still relevant information but worth checking with Saga direct.  We were on Spirit of Adventure on the 20 th January cruise and during that cruise Ship Reception filled and printed all the necessary passenger locator forms for all Pax and delivered them to our cabins the day before each port where they were necessary.  I would check with Saga if they are still doing that as it saved a lot of bother.   

Were also told to take all the paperwork with us each time we disembarked for a day visit but in our case no one ever checked anything at any port.
