Transfer cruise credits

I booked and paid for 2 cabins, we received future cruise credits to be used within 1 year and the credit was issued to each of the cruisers.  I remember reading there was a form that my kids could fill out to transfer their cruise credits to me, does anyone have a link to that form? I tried searching on the NCL site and cannot find it.
