NCL is freestyle cruising, you can go to MDR in shorts and t-shirt, but don't go in a bathing suit, be presentable. Only a couple of restaurant require gentleman to wear slacks (Le Bistro, Palomar). The drink package is one of the best out there, but you don't have to drink the whole bar on the first day, it will be open the next day. Don't be a lounger/pool chair hog by putting your belongings on chair at 8am and returning at 3pm. You pay a daily gratuity fee per person per day, but if you get good service, it doesn't hurt to tip a little extra. Yes we are in our early 50s, no medical conditions, but like to have the travel insurance because our medical does not cover us if something happens outside the US. Its also a lot cheaper than buying a policy every time you book a trip.

Depending on your needs and how many times you travel a year, you might be better off with an annual insurance policy. I have an annual policy with Allianz for 2 people, it has great medical coverage and covers all trips including flights within 365 days. Do your research and read limitations of coverage, for me it is a lot cheaper than buying insurance for every trip.
