There are some very high-end policies that might, so you'd have to check yours, but generally it won't. And while some might cover emergency care, that would be only for smaller things, and not for medical evacuation either off the ship to the nearest country, or better yet, off the ship and to a US hospital. There are a lot of other reasons why people buy travel insurance, though. Some of them are: air flight complications like cancellations, delays, missed flights; hotel stays before/after the cruise; travel to the port by other means (like driving to the airport and getting into an accident); job loss; illness; death or illness of someone in your immediate family that would cause you to cancel your cruise; and a ton of other things. So you have to consider your risk aversion -- how much are you willing to forfeit (pay) if you don't go on the cruise, or have some problem while on the cruise...and buy a policy that covers that.
