Just fill out the little card your cabin steward leaves you, specifying what you want and the hour you want it delivered, and hang it outside your cabin door before you go to sleep. A hint: if you tick delivery between 08.00-08.15, they'll probably knock on your door at 07.55. For some reason they're usually early. No tip is expected. But be fast, according to the 2012 catalogue Breakfast in the Cabin will only be FOC for suite passengers, normal mortals will have to pay. Difficult question to answer. The doing away of the free breakfast was first mentioned in the 2012 catalogue and Costa cruise year is normally November to November.
