Health Questionnaire Before Embarking

As my husband and I will be going on our first cruise next Saturday from Brisbane I wondered what questions are asked on the health questionnaire. Is it about previous illnesses/medical conditions or symptoms you may have at that time?

It just asks if you have been ill in the last 24-48 hours.
