If we fly from a US city, let's say Pliladelphia, to San Jaun, do we have to pass through customs at any point ?

You would go thru customs on you entry into the US.

If you fly to San Juan for a cruise then it is my understanding that you will clear US customs upon your return to San Juan as it's a US territory. If you fly to San Juan, stay there for a week and then return I don't believe that you clear customs at all.

Thanks for the information. I assumed as much , but my travel companion thought we would have to get to the aeroport 3 hours early---- and you know what can happen if we assume.

You do not clear customs flying to or from San Juan. The only thing you do in San Juan is put your bag through the USDA scanner to check for fresh fruits/flowers/nuts etc at the airport. Dont lose your sticker before you get to the xray line. it's the same process/line you go through in the US. You would on the ship clear customs at debarkation but not the airport.
