The form is rather silly IMO. Does anyone ever answer yes? The question should really read "do you want to be denied boarding without a refund? Why do you ask this? Will it make the difference whether you take the cruise or not? If they have one fill it out if they don't then they don't. I was actually sick when we boarded but I had been sick for a while and had been on medication so I was no longer contagious (it was just a sinus infection). The 2 or 3 prior to that, the check in people just asked if anyone was sick or had symptoms. It had been awhile since we actually had to complete the form prior to Ebola outbreaks.
