Yes. Yes and yes (split up in the sense that the three levels of the MDR have three different themes and names but all are part of the MDR).

I don't believe they show the dining times on the website until you get farther into the booking. You could call and find out. Generally, the cruises from Ft. Lauderdale have times of 6pm and 8:30pm.
