Vaccine Card -- Married name change .. what to do ?

So my wife's vaccine card has her maiden name on it.  The place where she originally got it is out of business.  What do we do ? Just bring a marriage license to show the original name ?

Yes, you'll need a "connecting" document to show the name change.

1)  Connecting document.

 

2)  Go to your current pharmacy with the connecting document and card and see if they will issue you a new one in your married name.

People from a Canadian province have posted here about this issue because their province requires all medical records to use birth names instead of married names.  Because of this, their maiden name is on their vaccine cards.  They have to bring a marriage license to act as a bridging document.
