Prior to covid, just changed the cases. I don't know what they do now. The extra pillows would take up tons of room. We are planning to bring our My Pillows with us (if we decide to do our cruise in January) so we can wash them when we come home. We have a couple travel ones. I was told that they are cleaned after each cruise and that we needed to wait until later in the day. He did swap them out for us. I am not sure if they are still doing this but I hope so. i always do a Lysol and Clorox wipe of the room as soon as we board.
