Her original vaccine card had her name before we were married but the health department updated her name. SG65CB Posted June 4, 2022 #7 Share Posted June 4, 2022 This happened a few months ago with a member of our party but in reverse, old passport with maiden name, vax card with married name. Booked on cruise with maiden name to match the passport. It caused a lot of worry. Brought the marriage license but didn't need it. At the port handed over the vax card without saying anything to see what would happen. They accepted it. We think the matching first name and date of birth was good enough to get it accepted.
