Annual Policy Question

I have an annual policy from Allianz Premier. It says it only covers trips of 45 days or less. I am taking a cruise in September that is 44 days plus a few more for flying. Can a rider be had to extend that, or should I look for another carrier? The policy costs $485 per year.

I’m curious about this because I’m paying a fortune to Allianz to insure four cruises this year. I am over 65 and have secondary conditions, plus want a lot of coverage for ship evacuation by helicopter and repatriation in case of a medical emergency. My TA says their annual policy doesn’t cover secondary conditions
