If we are on medicare with have a secondary do we still need medical travel insurance if we are from US?

You don't need medical travel insurance to travel to Hawaii. You could certainly elect to get it, but it is in no way required unless it is a specific requirement of your particular cruise line.

You might want to insure your cruise against a last minute cancelation due to a medical emergency that your medical insurance will cover.

you also might want to consider the possible expense for transport home or off the ship in case of accident or unexpected medical needs
