Heathrow to St James park by tube

St James’ Park station doesn’t have elevators or escalators so you’d have to use stairs there. If that’s a deal-breaker, just get off at the previous stop Victoria, where there are elevators, and walk or take a cab from there.

 

Otherwise it’s an easy journey - Heathrow to Barons Court on the Piccadilly line, then a simple cross-platform transfer to the District line to St James’ Park (or Victoria).

To add to the excellent advice from @gumshoe958, the line at St James’ Park is only just sub-surface, not deep. There are just two flights of stairs (about 30:steps in total) to ground level and they are relatively wide and shallow staircases compared to some on the network. 
 

So not horrendous with luggage, given no mobility issues.
