In order to process your request, please fill out the attached form and respond to this email." The e-mail makes no reference to the online form I filled in or any issue with the information in it, so I've gone back to them to say I've already completed it electronically, as I dislike repeating myself especially if there's no good reason given. Just to update, I went back to say I'd already done the onlien form and the next e-mail I had from them was to say the OBC was applied. No idea why they supplied me the PDF form when they clearly already had the required information. If that were the case for our accounts, I would resubmit the form. (Also, you should have received a case number if they received your document!)
