Rare 1025cruise Posted June 11 #2 Share Posted June 11 First, check with your cruise line. I believe Royal has a specific form that needs to be filled out, versus creating your own. Otherwise, just a letter signed and notarized by both parents giving you permission to take the child on the cruise and out of the country should be sufficient. We tried to be very complete in our letter, which gave us permission to - drive from our home state to our port city, then back again. - spend the night in ___ hotel the night before the cruise. - board ___ ship. - exit the ship for the day at islands ___, ___, and ___. - use our own authority for emergency medical treatment.
