If you picked the beverage package as a free at sea perk, you will be upgraded to the Premium Plus package. Bottled water, coffee in the restaurants, any alcoholic drink by the glass, as well as certain bottles of wine in restaurants are the upgrades from the included free package on the cruise.
I am also going to Cuba on the Sky and spoke to an NCL rep to make sure.

I booked through Expedia and it took a little over a day to show up on NCL website.

If you choose the upgrade to Premium Beverage, you get more choice of top shelf liquor, more wine options, some bottles of wine at dinner, bottled water, juice, and LaVazza coffee in restaurants. The coffee house drinks are still extra.

Visa is $75 per person. I was able to get it through NCL. See if Carnival will do it, or go through the Embassy.

I sailed on the Breakaway 4 years ago and dined at Ocean Blue. The most memorable part of my meal was the side dish of charred broccoli. Truly delicious and I've made it at home ever since.

That’s there for them to offer shuttle service from the airport to the cruise terminal and back. If you’re not interested, no need to complete that part of check in.
