Besides hepatitis A&B through the unsafe water, (the vaccine also protects you from other ways of catching it) there is also the less serious Montazuma's revenge from bad water. Unless the drink is sealed, or we know it is a safe destination we do not drink it. For the cost of a bottle of water, I would not risk it. They sell water as you leave the ship anyway. Ice cubes - again, unless you know the source of the water used, I would not use them. If the locals are drinking it, and not dying off, than it is safe. However, water is not pure, and so it has different minerals in it, based on where it is. With that in mind, your body, may reject it, to a certain degree. This goes the same for food. It might be tough to know, if it was water, your food, or some snack.
