Name of cruise line & ship, and name of port.

If just the name of the port doesn't work,  put the address of the (or a ) port terminal
 

It shouldn't be desperately important - your ESTA lasts for two years and you won't be required to update contact details on future visits, which makes the question pretty pointless. :classic_rolleyes:

Typically it can be anytime between now and the sailing date.  There is no specific time that you will be advised of your stateroom number with a GTY.  In our experience, however, we have had them assigned usually 2 weeks or so prior to sailing.
