Milk shakes and other beverages come with an extra cost. On the Quantum class cruise ships, Johnny Rockets does not have a cover charge and instead has an a la carte menu, where each item on the menu has an individual cost. No one gets a BOGO for JR any more. Not for quite a while. And the price is higher. It's not the same on every ship either. I think that it's $9.95 on Freedom. On some ships, this includes a soda with your meal. The only Quantum class ship with a Johnny Rockets is the Anthem. And yes, it has a la carte pricing. Go up to the top of this page and use the "search" box. I'm D+ and never had a free meal at Johnny Rockets. We always received a 2 for 1 shake. So there are burger joints more expensive than JR, still not gonna eat there. Place at Sands was called 'Mo Burger, saw no one buying anything.
