Spanish travel health form

The form is only needed if arriving by air. We called the Health Ministry and they confirmed this.

Buried in all the text on their website is this:

 

Passengers from 12 years of age, who arrive in Spain by  SEA  from countries that DO NOT belong to the European Union or are NOT considered Schengen associated countries must  be in possession of one of the required health certificates (vaccination, recovery or diagnostic test), whether it is an EU CCD/EU Equivalent or another type, without being required to complete a health control form.

We arrived by air yesterday.  We flew in from AMS after a 5 day stay there. We are US citizens.  We began in the US.  We had everything filled out but no one asked for it. 

They might not check for flights originating in Europe. They will check for other flights, like from the US or Canada.
