Does anybody have a picture of the health questionnaire. Trying to understand the time frame for close contacts, etc. 

 

I read you can’t be positive within ten days of sailing however, not sure what the other requirements are. Just trying to understand the requirements and expectations.

Back in December it was no close contact to anyone with covid within 14 days.  Pretty sure the question was the same in March.

Some of the questions are the same as they have asked for years, recent coughs, etc. And of course are you more than 23 weeks pregnant!   For many that's a funny question.   The questionnaire will show up on the app about 24 hrs before your checkin time.  Each person has to answer. Obviously answer carefully or you will have to answer more questions at the pier.

Somebody you know tells you they tested positive and you were in close contact with them during that time. The definition of close contact has evolved so not sure what the latest definition is however, I suspect when you know you know.
