I'm not sure what specific context you refer to, but, documents like your reservation confirmation list Lastname Firstname Middle.  Salutations in communication that I received were either "valued guest" or no salutation, except the upgrade mails which used Firstname Lastname.

For our November cruise, NONE of the communication used that format.

Not sure what you are asking. But correspondence usually have the name on your reservations which need to match your passport and travel docs. You don’t want nicknames on your reservations because you may be denied boarding.
