If you book a spa service such as a massage, do you get access to the thermal suite for some period of time before your service, or does thermal suite access always need to be purchased separately?

Always separate. Booking a spa service does not get you access to the thermal suite.

A good idea is to visit the Spa on embarkation day to verify your Spa reservations.  Two times, we discovered the ship didn't have a record of our appointments and had to shuffle some things around. (I learned this from a Spa Rep who was doing the meet and greet stuff during embarkation)
