Passport City/Authority?

Your passport should say on the first page where all of your information is which city or authority. It should be near the bottom of the page after your personal information.

When filling out the passport info w/the cruiseline, it asks for the city/authority. Is it United States and then the authority is Department of State?

Just put in U.S. State Department.....that's who authorizes the issuance.

I would go with whatever is listed on the data page; in my case, it is National Passport Center. I suspect that if someone gets their passport done in person _at a passport center_ rather than by-mail (or through the Post Office or county which is going to mail the apps anyway) it may say something different, like "New York" or "Philadelphia"

I think it all depends on where you get it from. Both my mom and I got ours in California, but she's in the SF Bay Area and I was in Los Angeles. Mine says Los Angeles County and hers says National Passport Center. All are correct.
