GloriaF Posted July 17, 2014 #1 Share Posted July 17, 2014 Hi, I have used the Search function but came up with an expired form for sailings through July 31, 2014 and reservations booked by Feb 28, 2014. I printed it before I noticed the expiration dates. We sail Aug 9, 2014, and just booked yesterday. Can someone PLEASE provide me for a link to a current form? Thanks! JF - retired RRT Posted July 17, 2014 #3 Share Posted July 17, 2014 You don't even need the form...just FAX your certificate/proof of ownership with your date of sailing, ship name, booking #, e-mail, and phone # written on it. They ask what the phone number of the fax I used is and they look it up that way. They apply it instantly and send me an email confirmation. I've never used a form.
