Very very few spots offer day passes. For the hotels that do offer them on occasion, they usually only include access to the facilities and possibly a room, they do not usually include any food or drink.

For more info on beach days in Cayman, read post #2 in this thread

We were on Grand Cayman this past week and booked a day pass through resorts for a day. We booked the Westin, $199. for 4 of us [not bad when two couples split the cost]. Nice property, got a room to change, shower, and keep our belongs safe. When checking in, we were give 4 drink coupons to use at the pool bar [excellent drinks]. We had access to the property, beach towels, lounge chairs, pool, beach, umbrellas, and could charge our luncheon to the credit card provided at check in.

We've used the same web site to book day passes on other islands and feel it's a good option to avoid beaches where vendors dominate. The booking is for up to 4 guests, much less than a beach shuttle shore ex through the ship.

As an FYI, on Cayman beaches there are no vendors of the sort you're likely trying to avoid. It's one of the few islands where you won't be pestered non-stop to buy a coconut or get your hair braided.

Also, when you can bring towels off the ship and rent chairs, umbrellas, etc separately for much cheaper, the only advantage to the elusive day pass (they're only generally offered at slower times of the year) is the room. If you don't need that, then you can save a ton of money by just going to someplace like Royal Palms (entry fee ~$2 pp includes adults-only pool, restrooms/changerooms and shower).
