Each ship & port is little different - but in the US you have the choice to get your new sea pass card & get off for few hours or "somewhat" stay on board. I say "somewhat" because you will be asked to meet with all of the other B2B cruisers - be escorted off the ship - smile at the US CBP person, then get back on.

Typically - at least with US itineraries - you will be issued your SeaPass card for the second cruise at the end of the first. You will have to disembark and go through customs and then re-board, but this is typically done as a group and in advance of the next cruise general boarding. You can then elect to stay on the ship or disembark if you want to do some sightseeing. If you are changing staterooms your stateroom attendant will move your clothes for you and provide access to the new room once moved. If you get off the ship you would just have to re-board by 90 minutes prior to sailing and will have to go through the muster drill again.

Be prepared to have a delay in getting off the ship. After a transatlatic ends in the US, there are multiple inspections and CPB procedures with all crewmembers having to see government agents.

Don't plan on doing anything that day. As Bob says, loads of inspections etc as the ship hits the first US port. I'm assuming from your screen name you are in the UK. We found out all about this several years ago doing a Trans Pacific. Got to Honolulu (first US port) and everyone had to go through immigration checks on the ship. This totally mucked up our one and only day in Honolulu as we didn't get off the ship until nearly 12 (having arrived at 8). Just plan to stay on the ship and the staff will escort you off and on when necessary.

you haven't said which ship you are on but the Caribbean transatlantic cruisse sometimes stop at St Thomas last to at least get immigration out of the way. I did the Independence last year and it was almost a straight forward walk off the ship in the US. Also RCI will sometimes offer reduced cost tours to get you out of the way when there are coast guard inspections etc and you are B2B.
