We put our excess cash in the safe & only take what we think we will need into each port. Every trip we come back with unused cash & just re-deposit to the bank. Play slots or something for a while. Then go to the cashier and cash out your account. It will not show as a cash advance, but as "entertainment". This definitely works. I would definitely pull the lever a time or two to be safe, but it would be very easy to load $100 or more, play a penny or quarter machine once or twice and cash out. Alternatively, Schwab offers a no fee ATM card. http://content.schwab.com/web/retail/public/get-started/checking/ They will reimburse any ATM fees. I have one and they typically post within 1-2 business days. I have one and was going to test it on Carnival's ATM to see if I would be reimbursed the full $6 fee, but I used the casino trick instead since I knew that would work. In Europe most ATM's ask which currency you would like get. Every ATM we have used in the Caribbean dispenses U.S. dollars. On the French side of St.Martin the ATM have a U.S or Euro button.
