Health Questionnaire information

I would like to know what information is required on the health questionnaire,  which must be completed 24 hours before boarding. I have an existing health condition and want to know if any doctor note will be required. Need this heads up so I can get it, if necessary, before the day before boarding. Thanks.

I have not sailed yet but the questionnaire appearing in the app under check-in shows all questions regarding covid symptoms.  Since it is longer than 24 hours before I sail on July 24th, there could be another page to it I am not seeing but I believe what they want is to determine if you are demonstrating any possible covid symptoms on the day before boarding.  I imagine if you answer yes to any of them they will give you a covid test to determine your eligibility to board.

Interested to know about the diabetes question on the link Fouremco shared. My husband is type 2 diabetic (well controlled). Is this going to be a problem with Celebrity? any diabetics who have travel so far able to comment?

If all the questions are covid related… no problem for me. I thought the questionnaire might contain other non contagious health questions.

This is not what's currently being used.  The one being used is what's on the app and is asking about the basic types of covid type questions we have been answering for the last year in addition to the noro type questions of yesteryear.   No Dr notes required.

As I think already said, the questions are all Covid related.  The doctors note stuff and preexisting conditions was on a prior questionnaire. 

Again, things could easily change again and again; just as in the last 18 months.

Has anyone (who is reading this) gone to a port [Greece, Sint Maarten, Fort Lauderdale] for embarkation in June/July, 2021 -- and has successfully boarded without having previously submitted the aforementioned "health questionnaire" via an app?

 

In other words ...

~~ Can the questionnaire (in "hard copy") simply be filled out within the cruise terminal?  ...

~~ OR can the questionnaire be filled out, via a phone conversation with Celebrity, within the 24-hour window? ...

~~ OR is there a standard online "page" [accessible via a www.celebritycruises.com URL] at which the questionnaire can be filled out via a desktop computer?

 

Our reason for asking is this:

We will not be able to fill out a questionnaire (via app) before getting to a port.
