Wrong name on COVID card

Hopefully, if the 8 month booster becomes a reality, you might be able to get it done in California and have a record that you got the third dose.

Someone mentioned concern their card only had one signature. My card has no signatures, as issued by the Publix grocery chain in Florida. They provided a computer-generated paper strip glued onto the white CDC card - the strip had name, date of birth, manufacturer of dose, dates given, batch codes. 
 

By the way, DH took our cards to be laminated when an office supply company was offering to do them for free. He recently contacted HAL and received emailed confirmation that laminated cards would not be a problem. 
 

Our nearest Publix store told him today that they’ll just stick the 3rd booster shot sticker on the back of card rather than issue a new one - they are running short on the cards these days.

On my card, all they did is handwrite in the date, vaccine type and lot number, and told me to fill in my own name and date of birth.  It doesn't look official at all.  Probably lots of people have similar.

Second dose was given at a Walgreens in another part of Texas and they wrote my second dose of information on first card.  When we went home a few months ago “reading the writing on the wall” regarding our cruise.  Sent off a request from Texas for my records (hubby’s too).  What we received back was a one sheet piece of paper with the correct information on it, but honestly not really that official looking.  Sent all the above to our health department hoping to get a QR code in California, no such luck.  Texas, is currently not agreeing to “vaccine passports “.  As the days get closer I’m getting more concerned.  Any suggestions?  Any other stories of COVID card woe?

You could just start over.  Go get a new shot (and card) and 28 days later get the "second" dose and you're good to go. Assuming you have time before the cruise.

On a different thread in a different forum, I had an obstinate poster trying to discredit me when I posted that there is no line for a signature on the CDC Card.  The closest to such a thing is the last column which is where either the medical provider prints their name or location of the administration site.  Most cards do not bear any signatures at all, and the Publix stickers have all the information which was requested on the card.

I believe it is for those experiencing domestic violence, etc. to maintain address confidentiality.

I know right, lol?  But I only want 1 booster shot!  Guess that could be a whole new thread.  We sail 8 months exactly from our second dose.  Hopefully we can get booster at home before we sail to Mexico!

It correctly shows both of my doses, "Vaccine Type", "Date Received", and "manufacturer, Lot Number"

I've just spoken with someone at the Washington State Dept of Health about the MyIRmobile site.

 

According to the WA DOH the MyIR certificate is considered an ISS record as the information provided comes directly from the State database.

 

Still need verification from HAL on their part.

 

I spoke with a HAL representative today who conferred with two resolution specialists. They all agreed that a MyIR certificate ( https://myirmobile.com/ ) is classified as a government ISS record and would be accepted by HAL as proof of COVID-19 vaccination.

 

They were all aware this information was going to be posted here.

 

As to being accepted by other cruise lines or countries I make no such claim.
