If you do not use up all of your OBC during the first leg of your b2b sailing on the same ship, does it carry over to the 2nd week?

No.  You cannot get cash back.

You have to use it up. Had this issue on B2B. We used it for boozy gifts

I’m surprised nobody has mentioned cashing it out in the casino.

I was on a cruise in January 2022 and had alot of OBC... Guest services said if we didn't use it all, I could go to Cherry on Top and buy a carnival gift card with it then use it on another cruise.  Also, they coded about 25% if it as refundable... that surprised me but we actually got some of the cash back....

Also, it was mentioned about going to the Casino and being able to cash it out.  How does this work?

Just off a B2B, considered separate cruises, had OBC on first cruise, bought Carnival Gift Cards at Cherry On Top with it.

Yes, you can use it to pay tips.  To cash out in the casino, you put your card in a slot machine, follow the instructions to set up a PIN, "charge to your room" the amount of OBC you want, spin one or move, upload remaining credits to your S&S and take card to cage and ask to be cashed out.
