The best days I've ever had in Rome have been walking days.

Get up early and start walking.  🚶‍♂️

You can see Vatican City, Castel Sant'Angelo, Colosseum, Pantheon, Trevi Fountain and the Spanish Steps in one day if you are in decent walking shape and aren't in need of partaking of the associated indoor museums or tours.  This would include time to rest over a nice lunch and dinner.  If you feel the need, you can always take a taxi back to your hotel at the end of the day.  But walking the city is immeasurably more satisfying than a bus tour, and going with your partner/family/self is much better than with a group.

The Vatican/ Sistine Chapel is a must see, the lines can be really long.  Do your homework.

Totally agree with this observation.  Stayed for five days in a VRBO in that area and everyday we were watching people and drinking wine!

If you have time for a food tour, Eating Europe offers tours in various European cities.  We've toured with them in Rome, Prague, and Lisbon.  They offer two in Rome: Twilight Trastevere Rome Food Tour and Taste of Testaccio in Rome Italy.  We've done both and would say we enjoyed the Twilight Trastevere tour more.

A few years ago I took the taste of Testaccio and enjoyed it very much, it’s an area in Rome I had not visited previously

However, my question to Reveuse is, have you ever been to Rome? If not I would certainly get a private tour guide that could show you the main sights in an efficient manner.
