Very simply people can still end up in quarantine if they become ill, are tested for COVID by the medical center, and produce a positive test result. The cruise lines are unlikely to change until the CDC eliminates its "voluntary" guidelines requiring testing for boarding a cruise. The cruise lines all accepted the "voluntary" guidelines because they did not want to risk the potential negative publicity consequences of not complying. I think a substantial percentage of the public subscribes to the "cruise ship as COVID petri dish" belief and if there were a major outbreak on a ship not in compliance with the CDC guidelines it would have a serious negative effect on bookings on that cruise line.
