Maybe not but you can count on the information on the State Department's website (www.state.gov) since they issue passports.  Post #6 links to the Passport Agency in New York City.

I don't think it is anywhere near everyone.   More likely the exception.   Could be a hundred different reasons. Question is what can be done if it happens.   Sadly, it seems options are limited.
