Your broker (Schwab, Fido) can fax it for you. Reception at my Schwab office (Walnut Creek, CA) just asks which 'desk' it needs to go to (Cunard, HAL, Princess). Also you can call Princess and have it added to recently booked cruises as long as the fax from Schwab is within 6 months or it's been added to a cruise within 6 months. I just did.  CCL makes it easy. Not as easy as the DD214 but still convenient
