Is Civitavecchia a port where everyone must disembark?

It was the port for our B2B but I can't remember if we were required to get off. We did get off and spent the morning walking around. At that time we took the shuttle from the pier to the entrancegate and just walked from there. On the return we just took the green city bus to the stop close by the entrance gate and then the shiuttle to the ship. I believe the bus routes have been changed and are different now.

No, it’s just another port - other than some immigration (like coming from the EU to UK) Europe is not like the U.S.  as long as you are not going to a different country area (and even then - you just do immigration), it not like cruising in the U.S.

If you are on a turnaround day and have been in the EU, it is whatever you want it to be - stay on board - explore Civi or just decide that day.   There are no rules like in the U.S. if you are in the EU.  You were cleared when you boarded 👍
